Case Management

Commonwealth Clinical Services, Inc.'s Case Management Program draws on the expertise of the agency’s professionals with an extensive background in home care. Our inter-disciplinary team offers sensitive, caring and professional services to promote independent living and added assistance when family support is limited; the client no longer qualifies for certified home health services or requires additional services that are covered; assisted living services are limited; or when a coordinated, patient-focused approach to care is desired.

 

Commonwealth Clinical Services, Inc.'s Case Management works with you and your family to develop a care plan that will provide quality care given by any or all of the following services:

• RN Physical Assessment

• Physical Therapy Assessment

• Skilled Nursing Visits

• Facility Assessments

• Client Advocacy

• Care Plan Development

• Continuous Communication with health care team, family members, and physicians in the development and/or change in the plan of care, physician appointments, laboratory and diagnostic testing

• Communication and progress reporting to family and physicians
